Your First Visit
Upon your first visit, you’ll be warmly welcomed by our front office staff members. You will be asked to fill out new patient paperwork and once completed, we will walk you back to one of our comfortable patient rooms.

Please provide the following information during your first appointment:
- Your referral slip
- Any x-rays
- A list of your current medications
- If you have dental insurance, your dental insurance card or any applicable information. (Not all dental insurances provide cards.)
Please let us know if you have any medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are taking any medications (i.e. heart medications, aspirin, anticoagulant therapy, etc.).
